How much difference does it make? Does it result in a significant performance increase?

For some operations yes, for others you may see no change. Its like using TweakXP or GameXP etc if you ever tried any of those.

Older games/apps seem to benefit more than recent releases.

I always take all these "tune up your PC for free" sites with a pinch of salt. I followed the tweakguides for XP to the letter, made all the recommended adjustments and lost over 500 aquamark points.
